Overview
Nephrotic syndrome is a kidney disorder that causes your body to excrete too much protein in your urine.
Nephrotic syndrome is usually caused by damage to the clusters of small blood vessels in your kidneys that filter waste and excess water from your blood. Nephrotic syndrome causes swelling (edema), particularly in your feet and ankles, and increases the risk of other health problems.
Treatment for nephrotic syndrome includes treating the underlying condition that's causing it and taking medications. Nephrotic syndrome can increase your risk of infections and blood clots. Your doctor may recommend medications and dietary changes to prevent these and other complications of nephrotic syndrome.
Symptoms
Signs and symptoms of nephrotic syndrome include:
Severe swelling (edema), particularly around your eyes and in your ankles and feet
Foamy urine, which may be caused by excess protein in your urine
Weight gain due to excess fluid retention
Fatigue
Loss of appetite

Causes
Nephrotic syndrome is usually caused by damage to the clusters of tiny blood vessels (glomeruli) of your kidneys.
The glomeruli filter your blood as it passes through your kidneys, separating things your body needs from those it doesn't. Healthy glomeruli keep blood protein (mainly albumin) — which is needed to maintain the right amount of fluid in your body — from seeping into your urine. When damaged, glomeruli allow too much blood protein to leave your body, leading to nephrotic syndrome.
Many possible causes
Many diseases and conditions can cause glomerular damage and lead to nephrotic syndrome, including:
Diabetic kidney disease. Diabetes can lead to kidney damage (diabetic nephropathy) that affects the glomeruli.
Minimal change disease. This is the most common cause of nephrotic syndrome in children. Minimal change disease results in abnormal kidney function, but when the kidney tissue is examined under a microscope, it appears normal or nearly normal. The cause of the abnormal function typically can't be determined.
Focal segmental glomerulosclerosis. Characterized by scattered scarring of some of the glomeruli, this condition may result from another disease or a genetic defect or occur for no known reason.
Membranous nephropathy. This kidney disorder is the result of thickening membranes within the glomeruli. The exact cause of the thickening isn't known, but it's sometimes associated with other medical conditions, such as hepatitis B, malaria, lupus and cancer.
Systemic lupus erythematosus. This chronic inflammatory disease can lead to serious kidney damage.
Amyloidosis. This disorder occurs when substances called amyloid proteins accumulate in your organs. Amyloid buildup often affects the kidneys, damaging their filtering system.
Blood clot in a kidney vein. Renal vein thrombosis, which occurs when a blood clot blocks a vein connected to the kidney, can cause nephrotic syndrome.
Risk factors
Factors that can increase your risk of nephrotic syndrome include:
Medical conditions that can damage your kidneys. Certain diseases and conditions increase your risk of developing nephrotic syndrome, such as diabetes, lupus, amyloidosis and other kidney diseases.
Certain medications. Examples of medications that can cause nephrotic syndrome include nonsteroidal anti-inflammatory drugs and drugs used to fight infections.
Certain infections. Examples of infections that increase the risk of nephrotic syndrome include HIV, hepatitis B, hepatitis C and malaria.
Complications
Possible complications of nephrotic syndrome include:
Blood clots. The inability of the glomeruli to filter blood properly can lead to loss of blood proteins that help prevent clotting. This increases your risk of developing a blood clot (thrombus) in your veins.
High blood cholesterol and elevated blood triglycerides. When the level of the protein albumin in your blood falls, your liver makes more albumin. At the same time, your liver releases more cholesterol and triglycerides.
Poor nutrition. Loss of too much blood protein can result in malnutrition. This can lead to weight loss, but it may be masked by swelling. You may also have too few red blood cells (anemia) and low levels of vitamin D and calcium.
High blood pressure. Damage to your glomeruli and the resulting buildup of wastes in your bloodstream (uremia) can raise your blood pressure.
Acute kidney failure. If your kidneys lose their ability to filter blood due to damage to the glomeruli, waste products may build up quickly in your blood. If this happens, you may need emergency dialysis — an artificial means of removing extra fluids and waste from your blood — typically with an artificial kidney machine (dialyzer).
Chronic kidney disease. Nephrotic syndrome may cause your kidneys to gradually lose their function over time. If kidney function falls low enough, you may require dialysis or a kidney transplant.
Infections. People with nephrotic syndrome have an increased risk of infections.
Overview
Acute kidney failure occurs when your kidneys suddenly become unable to filter waste products from your blood. When your kidneys lose their filtering ability, dangerous levels of wastes may accumulate, and your blood's chemical makeup may get out of balance.
Acute kidney failure — also called acute renal failure or acute kidney injury — develops rapidly, usually in less than a few days. Acute kidney failure is most common in people who are already hospitalized, particularly in critically ill people who need intensive care.
Acute kidney failure can be fatal and requires intensive treatment. However, acute kidney failure may be reversible. If you're otherwise in good health, you may recover normal or nearly normal kidney function.
Symptoms
Signs and symptoms of acute kidney failure may include:
Decreased urine output, although occasionally urine output remains normal
Fluid retention, causing swelling in your legs, ankles or feet
Shortness of breath
Fatigue
Confusion
Nausea
Weakness
Irregular heartbeat
Chest pain or pressure
Seizures or coma in severe cases
Sometimes acute kidney failure causes no signs or symptoms and is detected through lab tests done for another reason.

Causes
Acute kidney failure can occur when:
You have a condition that slows blood flow to your kidneys
You experience direct damage to your kidneys
Your kidneys' urine drainage tubes (ureters) become blocked and wastes can't leave your body through your urine
Impaired blood flow to the kidneys
Diseases and conditions that may slow blood flow to the kidneys and lead to kidney injury include:
Blood or fluid loss
Blood pressure medications
Heart attack
Heart disease
Infection
Liver failure
Use of aspirin, ibuprofen (Advil, Motrin IB, others), naproxen sodium (Aleve, others) or related drugs
Severe allergic reaction (anaphylaxis)
Severe burns
Severe dehydration
Damage to the kidneys
These diseases, conditions, and agents may damage the kidneys and lead to acute kidney failure:
Blood clots in the veins and arteries in and around the kidneys
Cholesterol deposits that block blood flow in the kidneys
Glomerulonephritis (gloe-mer-u-loe-nuh-FRY-tis), inflammation of the tiny filters in the kidneys (glomeruli)
Hemolytic uremic syndrome, a condition that results from premature destruction of red blood cells
Infection
Lupus, an immune system disorder causing glomerulonephritis
Medications, such as certain chemotherapy drugs, antibiotics, and dyes used during imaging tests
Scleroderma, a group of rare diseases affecting the skin and connective tissues
Thrombotic thrombocytopenic purpura, a rare blood disorder
Toxins, such as alcohol, heavy metals, and cocaine
Muscle tissue breakdown (rhabdomyolysis) that leads to kidney damage caused by toxins from muscle tissue destruction
Breakdown of tumor cells (tumor lysis syndrome), which leads to the release of toxins that can cause kidney injury
Urine blockage in the kidneys
Diseases and conditions that block the passage of urine out of the body (urinary obstructions) and can lead to acute kidney injury include:
Bladder cancer
Blood clots in the urinary tract
Cervical cancer
Colon cancer
Enlarged prostate
Kidney stones
Nerve damage involving the nerves that control the bladder

Risk factors
Acute kidney failure almost always occurs in connection with another medical condition or event. Conditions that can increase your risk of acute kidney failure include:
Being hospitalized, especially for a serious condition that requires intensive care
Advanced age
Blockages in the blood vessels in your arms or legs (peripheral artery disease)
Diabetes
High blood pressure
Heart failure
Kidney diseases
Liver diseases
Certain cancers and their treatments
Complications
Potential complications of acute kidney failure include:
Fluid buildup. Acute kidney failure may lead to a buildup of fluid in your lungs, which can cause shortness of breath.
Chest pain. If the lining that covers your heart (pericardium) becomes inflamed, you may experience chest pain.
Muscle weakness. When your body's fluids and electrolytes — your body's blood chemistry — are out of balance, muscle weakness can result.
Permanent kidney damage. Occasionally, acute kidney failure causes permanent loss of kidney function, or end-stage renal disease. People with end-stage renal disease require either permanent dialysis — a mechanical filtration process used to remove toxins and wastes from the body — or a kidney transplant to survive.
Death. Acute kidney failure can lead to loss of kidney function and, ultimately, death.
Prevention
Acute kidney failure is often difficult to predict or prevent. But you may reduce your risk by taking care of your kidneys. Try to:
Pay attention to labels when taking over-the-counter (OTC) pain medications. Follow the instructions for OTC pain medications, such as aspirin, acetaminophen (Tylenol, others), ibuprofen (Advil, Motrin IB, others) and naproxen sodium (Aleve, others). Taking too much of these medications may increase your risk of kidney injury. This is especially true if you have pre-existing kidney disease, diabetes or high blood pressure.
Work with your doctor to manage kidney and other chronic conditions. If you have kidney disease or another condition that increases your risk of acute kidney failures, such as diabetes or high blood pressure, stay on track with treatment goals and follow your doctor's recommendations to manage your condition.
Make a healthy lifestyle a priority. Be active; eat a sensible, balanced diet; and drink alcohol only in moderation — if at all.
A new study by researchers from the George Institute for Global Health in New Delhi and the Postgraduate Institute of Medical Education and Research in Chandigarh found that nutritional Vitamin D supplementation improved vascular function and reduced inflammation in patients with early stage of chronic kidney disease.
Vitamin D, which is both a vitamin and a hormone, helps control levels of calcium and phosphate in blood and is essential for the formation of bones and teeth. The findings were presented at the ongoing American Society of Nephrology Kidney Week 2015 (November 3 - November 8) in San Diego, US.
"Vitamin D deficiency is common in patients with chronic kidney disease. Our study shows that simply identifying and correcting this abnormality has the potential to improve the outcomes in these patients," said Vivek Kumar, nephrologist at the Chandigarh institute and the first author of the study.
About one in 10 people suffer from chronic kidney disease around the world. Most of these patients are destined to develop premature cardiovascular disease, said Vivekanand Jha, leader of the study group. In a randomised controlled trial under direct supervision the researchers gave two doses of 300,000 units of Vitamin D to one group of patients 8 weeks apart whereas the patients in the other group received matching placebo.
Vitamin D is cheap and widely available, and through its favorable effects on inflammatory and immune functions, it has the potential to favorably influence the outcome in these patients, said the researchers.
"This study has shown improvement in a range of parameters in Vitamin D-treated patients. What makes this finding of special importance is that patients in both groups were already on optimal treatment to reduce cardiovascular risk, hence the benefit of Vitamin D was additive," Jha added.
About 70 percent of patients receiving Vitamin D demonstrated significant improvement in their vascular functions and improvement in biomarkers indicating reduction in the level of inflammatory and immune activation.
In the placebo arm, only 5 percent patients showed improved vascular function and there was no change in the inflammatory and immune markers. Neither the patients nor the study doctors were aware of which patients received which treatment till the conclusion of the study, the researchers said.
नेफ्रोटिक सिन्ड्रोम :
किडनीच्या या रोगानुके कोणत्याही वयात रुग्णाच्या शरीरावर सूज येऊ शकते, परंतु मुख्यत्वेकरून हा रोग छोट्या मुलांत आढळून येतो. योग्य उपचाराने रोगावर संपूर्ण नियंत्रण मिळविल्यानंतरही पुन्हा सूज दिसणे आणि ती वर्षानुवर्ष चालू राहणे हे या रोगाचे वैशिष्ट आहे. बऱ्याच वेळा पुन्हा पुन्हा सूज येण्यामुळे हा रोग,रुग्ण आणि त्याचे कुटुंबीय यांच्याकरिता चिंतेचा विषय होतो.
नेफ्रोटिक सिन्ड्रोममध्ये किडनीवर काय परिणाम होतो?
सोप्या भाषेत सांगायचे तर,किडणी शरीरात चाळणीचे काम करते. किडणीमुळे शरीरातील अनावश्यक पदार्थ व अतिरिक्त पाणी लघवीद्वारे बाहेर फेकले जाते.
नेफ्रोटिक सिड्रोममध्ये किडनीची चाळणीसारखी असलेली भोके मोठी होतात , ज्यामुळे अतिरिक्त पाणी व उत्सर्जी पदार्थाबरोबर शरीराला आवश्यक प्रोटीन्सहि लघवीवाटे बाहेर पडतात,त्यामुळे शरीरातील प्रोटीनचे प्रमाण कमी होते व शरीराला सूज यायला लागते. लघवीवाटे बाहेर जाणाऱ्या प्रोटीनच्या प्रमाणावर रुग्णांच्या शरीरावरील सुजेचे प्रमाण कमी जास्त होते. नेफ्रोटिक सिन्ड्रोममध्ये सूज असताना सुद्धा, अनावश्यक पदार्थ बाहेर फेकण्याची किडनीची कार्यक्षमता शाबूत राहते.अर्थातच किडणी खराब होण्याची शक्यता खूपच कमी असते.
नेफ्रोटिक सिंड्रोम कोणत्या कारणांमुळे होतो?
नेफ्रोटिक सिन्ड्रोम होण्याचे निश्चित कारण अजून सापडलेले नाही. श्वेतकणांमध्ये लिम्फोसाइटसच्या कार्याच्या अभावाने हा रोग होतो असे मानले जाते. आहारात बदल किंवा औषधांना यासाठी जबाबदार धरणे हे अत्यंत चुकीचे आहे.
नेफ्रोटिक सिन्ड्रोमची लक्षणे
- हा रोग मुख्यतः २ ते ६ वर्षाच्या मुलांमध्ये दिसून येतो. इतर वयाच्या व्यक्तींमध्ये ह्या रोगाची संख्या मुलांच्या तुलनेत कमी आढळून येते.
- सर्वसामान्य या रोगाची सुरुवात ताप व खोकल्याने होते.
- रोगाच्या सुरुवातीची खास लक्षणे म्हणजे डोळ्याखाली व चेहऱ्यावर सूज दिसणे. डोळ्यांच्या आजूबाजूला सूज दिसू लागल्यामुळे कित्येक वेळा रुग्ण प्रथम डोळ्यांच्या डॉक्टरांकडे तपासणीकरिता जातात.
- हि सूज जेव्हा रुग्ण सकाळी उठतो तेव्हा जास्त दिसते.ह्या रोगाची हि जणू ओळखच आहे. दिवस वर येतो तशी सूज हळूहळू कमी होऊ लागते आणि संध्याकाळी अगदीच कमी होते.
- रोग वाढल्यावर पोट फुगते,लघवीचे प्रमाण कमी होते, पूर्ण शरीराला सूज येते आणि वजन वाढते.
- कित्येक वेळा लघवीला फेस येणे, ज्या जागेवर लघवी झाली असते तिथे पांढरे डाग दिसणे अशी लक्षणे दिसून येतात.
- या रोगात लघवी लाल रंगाची होणे, धाप लागणे किंवा रक्तदाब वाढणे अशी लक्षणे आढळून येत नाहीत.
नेफ्रोटिक सिंड्रोम मध्ये कोणते गंभीर धोके उत्पन्न होऊ शकतात?
नेफ्रोटिक सिंड्रोममध्ये असामान्यरित्या दिसणारे गंभीर धोके म्हणजे पोटात जंतुसंसर्ग (Peritonits),मोठ्या शिरेत (मुख्यतः पायाची) रक्त साकाळने (Venous Thrombosis ) इ.
नेफ्रोटिक सिंड्रोमचे निदान
1 . लघवीची तपासणी
- लघवीतून अधिक प्रमाणातून प्रोटीन जाणे नेफ्रोटिक सिंड्रोमच्या निदानाचा सर्वात महत्वाचा संकेत आहे.
- लघवीतून रक्तकण,श्वेतकण किंवा रक्त न जाणे हाही ह्या रोगाच्या निदानाचा महत्वपूर्ण संकेत आहे.
- २४ तासात लघवीतून जाणाऱ्या प्रोटीनची एकूण मात्रा ३ ग्रँमपेक्षा अधिक असते.
- लघवीची तपासणी केवळ रोगाच्या निदानासाठी नव्हे तर रोग्याची उपचारपद्धती ठरवण्याकरिताही महत्वाची असते. लघवीतून जाणारे प्रोटीन जर बंद झाले तर उपचार यशस्वी झाला असे सिद्ध होते.
2 . रक्ताची तपासणी
सामान्य तपासणी: बऱ्याचशा रुग्णांत हिमोग्लोबिन , श्वेतकणांची मात्रा इ.ची तपासणी आवश्यकतेनुसार केली जाते.
निदानाकरता आवश्यक तपासणी: नेफ्रोटिक सिंड्रोमच्या निदानासाठी रक्त तपासणीत प्रोटीन (अल्बूमीन) कमी असणे व कोलेस्टॉल वाढलेले असणे आवश्यक आहे.सामान्यतः रक्त तपासणीत क्रिअँटीनिनचे प्रमाण सर्वसाधारण असल्याचे आढळते.
अन्य विशिष्ट तपासणी: डॉक्टरांच्या सल्ल्यानुसार आवश्यकतेप्रमाणे बऱ्याच वेळा कराव्या लागणाऱ्या रक्ताच्या विशेष तपासन्यांमध्ये कॉम्प्लिमेंट ,ए.एस,ओ.टाइटर ,ए,एन .ए.टेस्ट ,एड्सची तपासणी,हिपेटाइटीस ची तपासणी यांचा समावेश असतो.
3 . रेडीओलॉजिकल तपासणी
या तपासणीत पोटाची ,किडनीची सोनोग्राफी , छातीचा एक्सरे यांचा समावेश असतो.
नेफ्रोटिक सिंड्रोमसाठी उपचार: नेफ्रोटिक सिंड्रोमच्या उपचारांत आहाराचे पथ्य,विशेष काळजी आणि आवश्यक औषधे घेणे अत्यंत महत्वपूर्ण आहे.
1. आहारात पथ्य पाळणे :
शरीरावर सूज असल्यास व लघवीचे प्रमाण कमी असल्यास रुग्णाला पाणी व मीठ कमी घेण्याचा सल्ला दिला जातो. बहुतेक मुलांना प्रोटीन सामान्य प्रमाणात घेण्याचा सल्ला दिला जातो.
२. संसर्गावर उपचार व संसर्गापासून बचाव :
- नेफ्रोटिक सिंड्रोमचे विशेष उपचार सुरु करण्याच्या आधी मुलांच्यात जर कोणता संसर्ग झाला असेल,तर अशा संसर्गावर प्रथम नियंत्रण मिळवणे आवश्यक असते. नेफ्रोटिक सिंड्रोमने पिडीत मुलांमध्ये सर्दी ,ताप वगैरे प्रकारचे संसर्ग होण्याची शक्यता अधिक असते. उपचार चालू असताना संसर्ग झाल्यास रोग बळावू शकतो,म्हणूनच उपचार चालू असतात संसर्ग होऊ न देणे याची विशेष खबरदारी घेणे व संसर्ग झाल्यास त्वरित व ठाम उपचार करणे आवश्यक आहे.
3 . औषधांद्वारे उपचार
सामान्य उपचार :
- सुजेवर लवकरात नियंत्रण मिळवण्याकरता लघवी जास्त प्रमाणात होईल अशी औषधे (डाययुरेटिक्स) थोड्या काळाकरता देण्यात येतात .
विशिष्ट उपचार :
- नेफ्रोटिक सिंड्रोमला काबूत आणण्यासाठी सर्वात जास्त प्रचलित व परिणामकारक औषधे आहे प्रेडनिसोलॉन ! हे स्टेरॉइड वर्गातील औषध आहे. जर या औषधाने परिणाम झाला नाही तर इतर औषधांचा वापर केला जातो.
- प्रेड्नीसोलॉन लघवीतून जाणाऱ्या प्रोटीनवर नियंत्रण ठेवणारे परिणामकारक औषधे आहे.हे औषधे किती द्यायचे हे मुलाचे वजन व रोगाचे गांभीर्य लक्षात घेऊन डॉक्टर निश्चित करतात.
- हे औषधे किती काळाकरिता आणि कशा प्रकारे घ्यायचे हे तज्ञ डॉक्टर ठरवितात . या औषधाच्या सेवनाने बहुतांशी रुग्णांमध्ये एक ते चार आठवड्यात लघवीतून प्रोटीन जाने बंद होते.
- प्रेडनिसोलॉन औषधाचे दुष्परिणाम ( Side effects) काय असतात? प्रेडनिसोलॉन नेफ्रोटिक सिंड्रोमच्या उपचाराचे मुख्य औषध आहे.
1. परंतु या औषधाचे साइड इफेक्ट्सहि आहेत. या दुष्परिणामांचा प्रभाव कमी करण्यासाठी हे औषधे डॉक्टरांच्या सल्ल्याने व त्यांच्या देखरेखीखाली घेणेच योग्य आहे.
2. प्रेडनीसोलॉनमुळे थोड्याच काळात दिसणारे दुष्परिणाम :
अधिक भूक लागणे,वजन वाढणे, अँसीडीटी होणे,पोटात व छातीत जळजळने ,स्वभाव चिडचिडा होणे, संसर्ग होण्याची शक्यता वाढणे,रक्तदाब वाढणे इत्यादी .
प्रेडनीसोलॉनमुळे बऱ्याच काळानंतर दिसणारे दुष्परिणाम :
मुलांचा विकास कमी होणे (उंची न वाढणे) , हाडे कमजोर होणे,चामडी सैल पडल्याने मांड्या व पोटाच्या खालील भागावर गुलाबी चट्टे पडणे,मोतीबिंदू होण्याची भीती असणे.
- इतक्या विपरीत परिणामांना जबाबदार असलेले प्रेडनीसॉल औषधे घेणे मुलांसाठी फायदेशीर असते का?
1. होय. सर्वसाधारण हे औषधे जास्त प्रमाणात , बराच काळ घेतल्यानंतर याचा विपरीत परिणाम होण्याची भीती असते. डॉक्टरांच्या सल्ल्यानुसार योग्य प्रमाणात आणि कमी कालावधीकरीता हे औषधे घेतले,तर विपरीत परिणाम होण्याची शक्यता कमी व कमी काळ असते.
2. औषधे जेव्हा डॉक्टरांच्या देखरेखीखाली घेतले जाते. तेव्हा गंभीर व विपरीत परिणामांच्या लक्षणांचे निदान त्वरित होते व त्वरित उपचारही करून त्या परिणामांना कमी केले जाते किंवा थांबवताही येते.
3. तरीही रोगामुळे होणारा त्रास आणि धोक्याच्या तुलनेत , औषधाचे विपरीत परिणाम कमी हानिकारक असतात. म्हणूनच जास्त फायद्यासाठी थोडे विपरीत परिणाम स्वीकारण्याशिवाय दुसरा मार्गच नाही. बऱ्याच मुलांमध्ये उपचारांच्या दरम्यान तिसऱ्या किंवा चौथ्या आठवड्यात , लघवीतून प्रोटीन जाणे बंद झाल्यावरसुद्धा सूज राहते. असे का?
4. प्रेडणीसोलॉनच्या सेवनाने भूक वाढते. जास्त खाल्ल्यामुळे शरीरात चरबी जमा होते, ज्यामुळे तीन-चार आठवड्यात परत सूज आली असे वाटते.
- रोगामुळे येणारी सूज आणि चरबी जमा झाल्यामुळे सूज आल्यासारखे वाटणे या दोन्हीतील फरक कसा ओळखनार?
नेफ्रोटिक सिंड्रोममध्ये रोग बळावला कि, साधारणतः डोळ्यांखाली चेहऱ्यावर सूज दिसते.ती सूज सकाळी जास्त व संध्याकाळी कमी होते. त्याचबरोबर पायावरपण सूज येते. औषध घेतल्यामुळे नेहमी चेहरा , खाद्ये आणि पोटावर चरबी जमा होते, ज्यामुळे तिथे सूज असल्यासारखे दिसते. हि सूज दिवसभर समान प्रमाणात दिसते.
डोळे व पायाची सूज न्स्मे,चेहऱ्याची सूज सकाळी जास्त व संध्याकाळी कमी न होणे ,हि सुजेची लक्षणे नेफ्रोटीक सिंड्रोममुळे नाही हे दर्शवतात.
- नेफ्रोटिक सिंड्रोममुळे येणारी सूज आणि औषधाने चरबी जमा होऊन सूज आल्यासारखे वाटणे यातील फरक जाणून घेणे आवश्यक का आहे?
रुग्णाला योग्य उपचार ठरवण्याकरिता सूज येणे व सूज आल्यासारखे वाटणे यातील फरक जाणून घेणे आवश्यक आहे.
1 - नेफ्रोटिक सिंड्रोममुळे जर सूज आली असेल , तर औषधाच्या मात्रेत वाढ किंवा बदलाबरोबरच लघवीचे प्रमाण वाढविणाऱ्या औषधांची आवश्यकता असते.
2 - प्रेडणीसोलॉन ह्या औषधाच्या नियमित सेवनाने ,चरबी जमा झाल्यामुळे सूज असल्यासारखी वाटते, ज्यामुळे रोग आटोक्यात नाही किंवा रोग वाढलाय अशी काळजी करण्याचे कारण नाही . काही काळानंतर प्रेडणीसोलॉन औषधाचे प्रमाण कमी झाल्यावर , सूज काही आठवड्यात हळूहळू कमी होऊन पूर्णपणे जाते. अशा औषधांमुळे आलेल्या सुजेवर त्वरित सूज कमी करण्यासाठी कोणत्याही प्रकारचे औषध घेतले तर ते रुग्णाला हानिकारक होऊ शकते.
- प्रेडणीसोलॉन औषधाने फरक न पडल्यास ,इतर कोणती औषध उपयोगी पडतात?
नेफ्रोटिक सिंड्रोममुळे वापरण्यात येणाऱ्या अन्य औषधांमध्ये लीव्हामीझॉल मिथाइल प्रेडणीसोलॉन औषधाने ,सायक्लोफॉस्फेसाइड ,सायक्लोस्पोरीन , एम.एम.एफ.इ. औषधांचा समावेश आहे.
- नेफ्रोटिक सिंड्रोममध्ये मुलांच्या किडनीची बायोप्सी केव्हा केली जाते?
खालील परिस्थितीत बायोप्सी केली जाते.
1. रोगावर नियंत्रण मिळवण्यासाठी जास्त प्रमाणात आणि जास्त काळ प्रेडणीसोलॉन घ्यावे लागत असेल.
2. प्रेडणीसोलॉन घेऊनही रोग नियंत्रणात येत नसेल.
3. खूपशा मुलांमध्ये नेफ्रोटिक सिंड्रोम होण्यास ‘मिनिमल चेंज डिसीज’ हा रोग कारणीभूत असतो. परंतु काही मुलांमध्ये हा रोग ‘ मिनिमल चेंज डिसीजमुळे झाला नसेल अशी शंका असेल (लघवीत रक्तकणांची उपस्थिती .रक्तात क्रिअँटिनीनचे प्रमाण जास्त आढळणे , कॉम्प्लिमेंट(c- 3)चे प्रमाण कमी होणे इ . तेव्हा बायोप्सी करणे आवश्यक असते.
4. जेव्हा हा रोग मोठ्यांमध्ये आढळतो, तेव्हा साधारणतः उपचारांपूर्वी किडनीची बायोप्सी करण्यात येते.

योग्य नियमनासाठी तज्ज्ञांनियमित तपासणी करून घेणे जरुरी आहे. तपासणीमधील संसर्गाचा परिणाम , रक्तदाब , वजन, लघवीतील प्रोटीनचे प्रमाण आणि आवश्यक असल्यास रक्ताची तपासणी केली जाते.या माहितीच्या आधारे डॉक्टर औषधामध्ये योग्य तो बदल करू शकतात.
- नेफ्रोटिक सिंड्रोम केव्हा बरा होतो?
योग्य उपचारानंतर बहुतांश मुलांमध्ये लघवीतून अल्युमिन जाने बंद होते आणि रोग थोड्याच काळात काबूत येतो. परंतु काही काळानंतर जवळजवळ सगळ्या मुलांमध्ये पुन्हा हा रोग व सूज दिसू लागते आणि अशा वेळी पुन्हा उपचारांची गरज भासते. जसजसे वय वाढते, तसतशी रोग पुन्हा उपचारांची गरज भासते. जसजसे वय वाढते, तसतशी रोग पुन्हा उपटण्याची प्रक्रिया मंदावते. ११ ते १४ वर्षादरम्यान बऱ्याचशा मुलांमध्ये हा रोग पूर्णपणे बरा होतो.
नेफेरिटिस (मूत्रपिंडदाह)
हा रोग तीव्र व दीर्घकालीन अशा दोन स्वरूपांत नजरेस येतो. दोन्ही प्रकारच्या दाहांचीं साधारण लक्षणेंः- लघवींतून आल्ब्यूमिन नांवाचा सात्त्विक पदार्थ जाणें व त्याबरोबर रक्त व मूत्रनळयाच्या खरपुडया जाणें, जलोदर, रक्तशय व रक्तवाहिन्या यांत होणारा बदल, नेत्रांतील विकृति व मूत्रविषशोषण हीं आहेत. या रोगाचा पूर्ण शोध इंग्लंडांतील डॉक्टर ब्राइट यानें लाविला.
तीव्र दाहाचीं लक्षणें:
सर्दीमुळें प्रथम थंडी वाजून मोठा ताप योते, व डोकें दुखतें. चैन पडत नाहीं, घेर्या येतात; आणि उलटया होतात. कुशींत आणि पाठींत मूत्रपिंडाच्या ठिकाणीं वेदना होऊं लागतात. वृषणास सूज येऊन पीडा होते व त्यांतील गोळी कधीं वर चढते. कालचाल केल्यानें दुःख जास्त वाढतें. तहान फार लागते व शौचास अवरोध होतो. पुष्कळ वेळां लघवी करण्याची इच्छा होते परंतु ती अगदीं थोडी आणि अति लाल अगर काळसर लाल रंगाची होते.
रोगकालात पुढील उपद्रव उद्भवण्याची शक्यता असते :
न्यूमोनिया, मूत्रपिंडदाह, मध्यकर्णशोथ (बहिरेपणास कारणीभूत असतो), लालापिंडशोथ, ऊरुरक्तवाहिनीक्लथन(मांडीतील रक्तवाहिन्यांतील रक्ताची गुठळी बनून रक्तप्रवाह बंद पडणे), पायातील बोटांचा कोथ (बोटातील मऊ कोशिका समूहांचा नाश होणे).
निदान :
टिकून राहिलेल्या ज्वरामुळे विशिष्ट पुरळ उठविण्यापूर्वी आंत्रज्वराची शंका येण्याची शक्यता असते. ⇨ मस्तिष्कावरण शोथ, देवी, कांजिण्या, इतर प्रकारचे रिकेट्सियाजन्य रोग, इन्फ्ल्यूएंझा इ. रोगांशी या रोगाचे काहीसे साम्य असते. विशिष्ट रक्तपरीक्षा निदानास उपयुक्त असतात. यांपैकी पूरक-बंधी परीक्षा आणि व्हाइल-फेलिक्स परीक्षा नेहमी उपयोगात आहेत. पूरक बंधन करणारे प्रतिपिंड [प्रतिपिंड] रोग्याच्या रक्तात आजाराच्या सातव्या ते बाराव्या दिवसापासून तयार होतात. बाजारात तयार मिळणाऱ्या विशिष्ट प्रतिजनाबरोबर [ प्रतिजन] यांची प्रतिक्रिया तपासून हे प्रतिपिंड ओळखता येतात व रोगनिदान करता येते.
ऑस्ट्रियन वैद्य ई. व्हाइल आणि प्राग येथील सूक्ष्मजंतुशास्त्रज्ञ आ. फेलिक्स यांनी १९१५ मध्ये पोलंडमधील या साथीच्या रोगावर संशोधन करून निदानास उपयुक्त ठरलेली परीक्षा शोधली व ती त्यांच्या नावानेच ओळखली जाते. प्रोटियस व्हल्गॅरिस नावाच्या सूक्ष्मजंतूंचे समूहन करू शकणारी विशिष्ट समूहके (प्रतिजन द्रव्ये) या रोगाच्या रुग्णाच्या रक्तद्रवात असतात. या व्हाइल व फेलिक्स यांच्या शोधावर ही परीक्षा आधारलेली आहे. रोगाच्या सातव्या ते अकराव्या दिवसांमधील रक्तद्रव आणि प्रोटियस सूक्ष्मजंतूंचा विशिष्ट प्रकार (प्रलापक सन्निपात ज्वर निदानाकरिता ओ-एक्स १९ प्रकार) यांच्या मिश्रणातील सूक्ष्मजंतू समूहनाची तपासणी करतात.
रोगनिदान
मूत्रपिंडदाह या आजाराची लक्षणे सहज कळून येतात. यात चेहऱ्यावर सूज (विशेषतः सकाळी), लघवीचे प्रमाण कमी होणे, लघवी लालसर होणे, धुरकट लालसर लघवी होणे ही प्रमुख लक्षणे आहेत.
अशा मुलांमध्ये चेहऱ्यावर सूज, लघवी कमी होणे या खाणाखुणा दिसतात. त्यांना ताबडतोब डॉक्टरकडे पाठवणे आवश्यक आहे. तपासणीत अशा रुग्णांचा रक्तदाब वाढलेला दिसतो. लघवीत प्रथिने व रक्तपेशी दिसतात. एकूण लघवीचे 24 तासांतले प्रमाण नेहमीपेक्षा कमी (कधीकधी अर्धा लिटरच्या आसपास) आढळते. याबरोबरच कधीकधी ताप, पोटदुखी, उलटी, डोकेदुखी, इत्यादी त्रास आढळतो. डॉक्टरकडून योग्य उपचारानंतर बहुधा हा आजार आटोक्यात येतो. रोगनिदान होण्यात उशीर झाला तर मात्र धोका संभवतो.
तोंडावर सूज येऊन, पावलावरही सूज आली असल्यास मूत्रप्रवृत्ती होण्यासाठी पातळ औषधे देण्यापेक्षा सुंठ, मिरी, पिंपळी, आले यांचा वापर उपयोगी असतो. पुनर्नवासव दोन चमचे + दोन चमचे पाणी, दोन वेळा, सात ते दहा दिवस देतात. त्रिकटूचूर्ण दीड ग्रॅ. ते दोन ग्रॅम सकाळ-संध्याकाळ तीन दिवस द्यावे; पचनाची शक्ती कमी असल्यास याचा उपयोग होतो. हाताच्या अंगठयाइतके आले व तेवढाच गूळ सकाळी रिकाम्या पोटी घेणे (7 ते 14 दिवस). हे अगदी थोडीथोडी सूज असताना चांगले उपयोगी पडते.
